Instructions to the Teacher

Student Scan Forms have been preprinted with the necessary information (Student name, Teacher name, Course name, Section number, Period number, Room number, Test ID number, Student ID number) for all students enrolled in a class section. To accommodate students with schedule changes a number of additional blank Scan Forms are available. The teacher is responsible for filling in completely the information on the blank Scan Forms. The Test ID and Student ID numbers should be indexed on the left side without leading or trailing zeros unless they are a part of the number. Do not bubble in blank spaces after the numbers.

The students are NOT to write on the test. Test maybe used by several classes. Provide scratch paper for each student if needed. A #2 pencil must be used to bubble in the Scan Forms.

Upon completion of the test place all Scan Forms for each class section in a separate envelope, which was provided, and returned to the Ponca City Public Schools Central Office for scoring. Please, DO NOT place more than one class section in the same envelope. On the front of the envelope fill in the number of students completing the test and the date the test was administered.

Unless otherwise instructed, place all class Test Sets in the envelope provided for each teacher and return to the Ponca City Public Schools Central Office.

Please READ EVERY QUESTION. If you have some knowledge of the answer then select the correct answer but DO NOT QUESS. Leave the answer blank.

Use a #2 pencil to bubble in the circle on the Scan Form that matches the letter of the answer you have chosen. PLEASE MAKE YOUR MARKS DARK and ERASE ANY EXTRA MARKS. Mark only ONE answer per question. Please DO NOT WRITE ON THE TEST. Use your scratch paper for any work you need to do.
